Ivermectin inhibits the chloride channels of helminthic parasites and has been shown to have clinical efficacy for the treatment of onchocerciasis, strongyloidiasis, and ectoparasitic infection (e.g., scabies).3 The in vitro antiviral activity of ivermectin has been explored against a spectrum of viruses, including severe acute respiratory syndrome coronavirus 1, human immunodeficiency virus, dengue virus, Zika virus, yellow fever virus, West Nile virus, Hendra virus, chikungunya virus, Semliki Forest virus, Sindbis virus, and avian influenza virus.4 More than 60 randomized trials of ivermectin for the treatment of Covid-19 have been registered, and findings have been reported for as many as 31 clinical trials.5 The results have been discordant, and various review groups interpret the evidence differently — some advocating for benefits of ivermectin, and others reticent to conclude a benefit.6-8 However, most trials have been small, and several have been withdrawn from publication owing to concerns about credibility.9 To evaluate the efficacy of ivermectin for the prevention of progression of Covid-19 resulting in hospitalization among outpatients with SARS-CoV-2 infection, we conducted a randomized, placebo-controlled, adaptive platform trial (TOGETHER) in the state of Minas Gerais, Brazil. Although various interventions were assessed in this trial, we report here on the use of ivermectin at a dose of 400 μg per kilogram of body weight for 3 days as compared with placebo.
